Provider Requirements:

• Board Certification(s): ABEM, BEEM, ABFM, ABIM with 1 year Post Grad Training
• 3rd year residents are accepted - 2 years of case logs are requested, not required
• Life Certs: ACLS, PALS & ATLS required- ABEM EXEMPT

License Requirements:

• Must have an actual MS state license; will accept pending DEA
