Provider Requirements:
- Must have MI license and DEA in hand.
- BCIM or FM
- PALS, ACLS required
Facility Details:
- MD Day need; MD Night Need – Days and nights required- MUST do nights.

- Hospital Beds: 49
- ICU Beds: 6 open
- Codes: Yes-procedures required. (Training & Proctoring Available): central lines, intubations, and arterial lines nes, intubations and arterial lines
- Rapid response- yes
- Procedures: yes
- Backup: Comprehensive
- Shift Times: 12h Shifts (7a-7p; 7p-7a)
- 2 physicians on at night with tele backup
- 60-day credential timeline
