Provider Requirements:

• Boards Required: ABA
• Life Certs Required: ACLS, BLS, PALS
• License Requirements: Active TX license
• Required Procedure Exp: general anesthesiologist (not pediatric), comfortable with regional anesthesia. No cardiac cases.
• Credentialing Timeline: 60 days

Facility Information:

• # ORs: 5
• Physician Schedule: M-F; 10 hours is acceptable
• CRNA Schedule: -looking for 2 candidates, 3 weeks per month minimum
• Call Required/Schedule: In-House
• #Locum Shifts Avail: Need is for 2 candidates, 3 weeks per month minimum
